Asset hand-over form format for employees, with a free template
An asset hand-over form is the page an employee signs when the company gives them a laptop, a phone or an access card. Here is what it should contain, a format you can copy, and a Word file you can download and edit.

What the form is for
The form records three things: what was given, who received it, and that they accepted the company’s rules for it. It is also called an asset acknowledgement form, an asset allocation form or a laptop hand-over form. The name does not matter; the content does.
You will rarely look at it on the day it is signed. You will look at it eight months later, when someone resigns and their laptop charger is “at home”, or a phone comes back with a cracked screen and nobody remembers what state it was in. Then the form is the only record of what they received and what they agreed to.
What to put in it
Keep it to one page. Everything an auditor or HR person would ask about later should be on it, and nothing else.
- Company name and a reference number, so you can find the form again
- Date of hand-over, and who in IT handed the items over
- Employee name, employee code, department, designation, office and reporting manager
- One row per item: asset tag, item and model, serial number, accessories, condition
- The asset policy the employee accepts, with its version or date
- A short declaration in plain words
- Signatures, names and dates of the employee and of IT
Details people get wrong
“Dell laptop” is not enough. Write the serial number from the sticker under the laptop, because that is what you will match against the device in the store room or in Intune later. If you label assets, add the tag too, such as LAP-0217.
List the charger, bag and dongle in the accessories column. Chargers are the item most often missing at exit, and without a line on the form there is nothing to point to.
Write the condition at hand-over: New, Good, or a short note such as “scratch on lid”. It settles the argument about damage when the item comes back.
The format
Copy it as it is, or download the Word file and add your logo.

Declaration
I confirm that I have received the items listed above in the condition stated. I have read the company’s asset policy (version ____, dated ________) and I agree to:
- use these items for company work;
- keep them safe and not lend them to anyone;
- report loss, theft or damage to IT within 24 hours;
- not open, repair or modify them myself;
- return them, with all accessories, on my last working day or whenever the company asks.
Employee signature, name and date
Handed over by (IT): signature, name and date
Writing the declaration
The declaration in the template covers what most Indian companies ask for. Adjust it to your own asset policy, and keep it short enough that people actually read it. If you plan to recover the cost of damage or loss from salary, check with your payroll or legal adviser before you write that in, because deductions from wages are regulated.

- Do not open, repair or modify them
- Return everything, with accessories, on the last working day or when asked
Getting it signed when people are not in the office
Paper works when the employee is sitting in front of IT. It breaks down when a laptop is couriered to a new joiner in Jaipur, or swapped for someone working from home. The usual workaround, emailing a PDF and asking for a scanned copy, means half the forms never come back.
An electronic acknowledgement fixes that: the employee gets a link, checks the items on their phone, reads the policy and types their name, and you keep a record with the time, IP address and device. It is not an Aadhaar eSign or a digital signature certificate, so if you expect a legal dispute over an asset, ask your lawyer what they need.
Keeping the records
Fill in a new form for every hand-over, including swaps and replacements, rather than editing the old one. Keep each form with the employee’s file at least until the items are back and the Full & Final is closed. When the items come back, record the return on a receipt the employee also signs, with the condition of each item.
Or let AssetVow send it for signature
Pick the items from stock and the person. They get a link, check the items on their phone, accept your policy and type their name. You keep a PDF with the time, IP address and device, a copy goes to them, their manager and HR, and reminders go out on day 2 and day 5 until it is signed.

Common questions
Is an asset hand-over form mandatory in India?+
No law prescribes one. Companies use it because it is the simplest proof of what an employee received and agreed to, which matters at exit and during audits.
Can the employee sign the hand-over form electronically?+
Many companies accept an electronic acknowledgement: a link sent only to the employee, a policy they accept and their typed name, with the time, IP address and device recorded. It is not an Aadhaar eSign or a digital signature certificate; if you expect a legal dispute over an asset, ask your lawyer what they need.
Should chargers and accessories be on the form?+
Yes. List them in the accessories column of the item they belong to, or on their own row. Chargers and dongles go missing at exit more often than laptops.
What if an employee refuses to sign?+
Ask what they object to; it is usually a clause in the policy. Note the date and the reason and involve HR. Handing over without any acknowledgement leaves you with no record later.
How is this different from an asset register?+
The asset register lists every asset the company owns. The hand-over form is one employee’s acknowledgement of the items they received. You need both, and each form should match the register.
